MM-SOP-C-001 - 24/7 Mental Health Crisis Line
The crisis telephone pathway will provide 24/7, open access telephone lines for urgent NHS mental health support, advice and triage. In providing an open access service, there should be no restrictions on who can refer as mental health crisis should be self defined. This would include service users currently open to secondary mental health care services. The aim of this crisis line is to ensure that qualified practitioners receiving calls are able, as part of the telephone screening process, to recognise, respond to, formulate and safety plan clinical risk in order to ensure patient safety is a primary objective of the team. The Mental Health Crisis Line will be staffed by Assistant Psychologists from across all Mid Mersey boroughs.

This procedure describes the processes that should be followed for the initiation and maintenance of anticoagulant therapy for a patient during their inpatient admission. It provides guidance on Low Molecular Weight Heparin, vitamin K antagonists and Direct Oral Anticoagulation treatments in relation to assessing eligibility, contraindications and cautions; consent and counselling; prescribing and care planning; administration and monitoring; discharge planning and obtaining supplies.
