These guidelines provide clear guidance in respect of the requirements of the same sex accommodation standards and establishes how the Trust will monitor and respond in the event of any exception to ensure the delivery of clean, safe environments which provide care that guarantees privacy and dignity. Mixed sex occurrence should be considered a never event. Mixing should only occur where it is clinically justified for all the service users affected.

MMD-POL-010 - End-of Life Care Policy
This policy and linked procedure relates to all staff caring for any adult patient over 18 years where that persons death is anticipated and can be planned for. This may include patients for whom the Trust provide a community nursing service and patients who have an expected death within Mid-Mersey inpatient settings. This policy and its related procedures do not relate to traumatic or unexpected deaths. Nor does it relate to the deaths of children who are under 18. This is available in the End of Life Care for Children Policy & Procedure.

There have been repeated instances when information sharing could have prevented a child being seriously harmed. When Serious Case Reviews have taken place it has been established that information about historical safeguarding concerns with regard to other children related to adults living with the child, had not been shared between agencies. A recurrent theme within the analysis of Serious Case Reviews is that it is vital for practitioners to share information to safeguard and protect children (Brandon et al, 2009)It is essential that Child Health Records of newborn children must include full details of all siblings, step siblings and other children up to the age of 18 and any adult who may live in the family home of the newborn.
