How do we reduce health inequalities for people experiencing homelessness?
A free community of practice event for Service Managers and Strategic Leads to consider best practice in supporting people's health needs and reducing the health inequalities for people experiencing homelessness.

You will hear from 3 guest speakers covering primary care, secondary care and palliative care and through continuous evaluation and reflective learning we will work to identify what works and build on this and think about opportunities to challenge current practice if appropriate.

Overall, the work will encourage a range of sustainable solutions to create a shared vision of collaboration at all levels across the system. The model will be working under the Design Council principles ‘to remove the barriers that create undue effort and separation, enabling everyone to participate equally, confidently and independently in everyday activities.’ The model of delivery will help to build relationships across the system at leadership, management, and operational levels

27th September 9:45am – 2:30pm
Hosted at YMCA Together, 15 Leeds Street, Liverpool, L3 6HU, Facilitated by Homeless Link, funded through System P.
