MMD-PROC-011 - Record Keeping Procedure
This Procedure is intended to ensure that records, both paper and electronic, are
managed, retained and destroyed in accordance with the General Data Protection
Regulation, Data Protection Act 2018, Freedom of Information Act 2000 and the
Records Management Code of Practice for Health and Social Care 2016.
This Procedure describes the processes to be followed when creating, referencing,
using version controls, naming and filing documents.

- Staff are advised to always check that they are using the correct version of any policies rather than referring to locally held copies. The most up to date version of all Trust policies can be found on this webpage.
- * Please note that following the acquisition of Calderstones Partnership NHS FT, South Sefton Community Services transaction and the Liverpool Community Services NHS Trust transaction and the North West Boroughs Healthcare NHS FT transaction, a number of policies are specific to staff transferring from these services as indicated.
During the COVID-19 outbreak the Trust took the decision to suspend the routine review of policy documents, although changes were made when necessary. Users are asked to note that the review date on any policy document is advisory and that all policy documents remain live / valid whilst they remain on the Trust’s website. The routine review of policies has now been resumed by the Trust in a prioritised manner.
Readers are asked to note that when reading policy documents if they come across the job title ‘Chief Operating Officer’, this should now be read as ‘Directors of Operations / Divisional Director’
