The guidance on this page relates to Mid Mersey division staff who transferred from North West Boroughs Healthcare on 1 June 2021. 

The Datix system is the reporting system used across the Mid-Mersey division to record incidents, risks, complaints, concerns and compliments.

Guide for running reports 

To request a new Datix account or changes to your existing account please complete the Datix Access Request Form which can be found on the document search.

Access the Incident reporting form

To request a new Datix account or changes to your existing account please complete the Datix access request form which can be found under the forms and guidance section of the Intranet. 

To report a serous incident you can do so by accessing the serious incident fast track form.

Please note it is vital that before reporting a risk on Datix you must first report the risk to your line manager who will take the decision to report on Datix.
